As the 2017 CHIP comes to a close, it is now time to look forward and prepare for the next Community Health Improvement Plan, the 2020 CHIP. With the data gathered from the 2019 Community Health Needs Assessment, we aim to create a new and comprehensive CHIP that addresses the current needs of the Greater Lowell community.
In order to achieve this task, we need to examine the accomplishments, gaps, and opportunities for improvement of last year’s CHIP. Provided on this page are surveys for each objective under each priority area. If you are from the Greater Lowell Area (Billerica, Chelmsford, Dracut, Dunstable, Lowell, Tewksbury, Tyngsboro and Westford) and you or your agency has done work around any of these priority areas, please take the time to fill out the surveys under the priority area you have worked on. Your input is critical to the development of our 2020 CHIP.
