Advancing Interoperable Social Determinants of Health

This half-day webinar will highlight the current state of social determinants of health (SDOH) screening tools in care delivery, including their role in newer care and payment models, and will describe efforts and available resources to increase the interoperability of SDOH tools, concepts and data elements.
Participants will come away with a better understanding of how to leverage health IT to support the use of SDOH in clinical practice and to improve clinical decision support and quality measurement, care coordination, and population health management.

Learn About Accessing Grants to Train Your Employees
The Massachusetts Workforce Training Fund provides resources for businesses to train current and newly hired employees. Its goal is to help improve employee skills and maintain the economic strength of the Commonwealth’s businesses.
