The Greater Lowell Health Alliance, Lowell Community Health Center, and Lowell General Hospital are hosting a Community Partnership Vaccine Day, at MVP Vaccination site, located at 1001 Pawtucket Blvd. on Wednesday, May 5th! We have set aside appointments from 8am to 12pm on that day and streamlined the sign-up process to make securing a vaccine easier for our community partners in Greater Lowell.
We need your help getting people to this low barrier vaccine clinic. We are happy to provide transportation if needed. We have exciting and important updates to share, as well as next steps!
We are thrilled to be hosting another low-barrier vaccine clinic for community members on MAY 5 from 8am to 12p. Those who helped us get people vaccinated on April 14th, we need your help getting them back for their second shot. We will have an additional 1000 shots available for people to receive their first shot of Pfizer. If you have clients or community members who need to receive their first shot, please collect their information on the form below.
Who is Eligible?
We will have the Pfizer shot available, anyone age 16 or over who lives, works, or studies in MA is eligible. People who are 16 or 17 must either be accompanied by a guardian/parent OR present a SIGNED Consent to Treat form, which can be accessed here:
https://www.mass.gov/lists/ma-consent-and-screening-forms-for-people-under-18-years-of-age
We also want to remind folks who received their first shot at our clinic on April 14th, should return for their second shot on MAY 5 between 8a and 12p. Please remind folks to bring their vaccine cards with them. If the card is lost, we can assist people with replacements at the site.
We want to make sure we have enough translators on site to assist everyone in their preferred language! To make that possible, we are asking our community partners to identify volunteers who can assist with translation on May 5. We can also provide support for your organization, please see our mini-grant application
VOLUNTEERS: Please send volunteers contact information to Lisa Taylor-Montminy: Lisa.Taylor-Montminy@lowellgeneral.org.
We cannot wait to partner with you for another successful community vaccine day!
